Please note that I am an “Out-of-Network” provider. We do not file insurance for my clients, however, we will gladly provide you with a completed insurance claim form so that you can be reimbursed directly. You may want to call your insurance company prior to the session to determine whether a pre-authorization is required and to clarify your benefits.
Cancellation Policy Notice:
The private practice has a 24 hour cancellation policy. There is a $75 charge for missed appointments and late cancellations. If you are unable to keep appointment, please call our office as soon as possible to reschedule.

201 Shannon Oaks Circle is located near the intersection of Kildaire Farm Road and Cary Parkway, directly behind the Chili's Restaurant. Turn into the parking lot for Chili's and proceed down the hill. Our building is at the bottom of the circle. My office is located on the second floor, Suite 200.
