Please arrive a few minutes early to your first appointment and bring the following with you:
- Completed New Client Questionnaire and Client Consent Form. You can download a copy on the printable forms page.
- Your insurance card for me to copy for those planning to use health insurance.
- Your payment for the initial one hour consultation
This practice accept checks, payable to “Orenstein Solutions, P.A.” as well as Master Card and Visa. A fee schedule can be found on the client consent form.
Insurance Reimbursement
Dr. Eve Fontaine accepts the Duke University, United Behavioral Health Plan. Duke University students who would like to utilize Dr. Fontaine's services will be responsible for a co-pay at the time of each session.
Please note that the psychologists at Orenstein Solutions, P.A. are Out-of-Network" providers for other insurance plans. If you choose to file insurance, our office will make it very easy for you. We will gladly fill out and mail your insurance claim forms to your home at the beginning of each month at no additional charge to you. Once you receive the claim forms in the mail, you simply mail the form to your insurance company, who in turn, should mail you a check.
We strongly recommend researching details about your mental health coverage. If you have a secondary health insurance company, you’ll need to find out that coverage and the interactions with the first coverage. It’s helpful to find out specifics of your mental health coverage with your insurance company(ies) by asking questions such as: Is there a deductible? How many visits per year are covered? What are the dates of the benefit year? Which services are covered (e.g., couples therapy, group therapy)?
